DIRECT NOTICE TO PARENT OR GUARDIAN CONCERNING COLLECTION, USE OR DISCLOSURE OF CHILD’S PERSONAL INFORMATION
The Children’s Online Privacy Protection Act (COPPA) requires operators of a website or online service to both give parents or guardians of children under age 13 direct notice of our information practices and obtain their verifiable consent, before collecting personal information from their children and we are providing this notice in order to obtain your consent to collect your child’s personal information.
- We have collected your online contact information for the purpose of obtaining your consent to the collection of your child’s information for the purpose of your child attending online dance classes with us.

- We wish to collect the following information from your child for the purpose of attending our online dance classes and to provide updates about those classes:
- Child’s email address (for the Zoom link or to contact the child about classes)
- IP address
- Photograph, video, or audio file containing your child’s image or voice
- Since the classes will be conducted on the Zoom online conferencing platform, your child’s face and voice will be visible to us and potentially others on the Zoom platform. You child will be able to speak during the class and may provide their name to us voluntarily. We will also record the class for later playback and instruction and will make the recordings available to other members of the class so that they may view the class over again, but only if you provide your consent. It will also contain your face and voice, if you are participating in the online classes.
- Please note that we will use this information to contact your child concerning their online classes with us and to engage in interactive instruction with your child and will not be combined with any other information collected from your child.

- If you do not consent within a reasonable amount of time, we will delete your online contact information from our records.

I/we realize that participation in dance classes and activities (the “Services”) could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Bachata Vida Dance Company, LLC dba Bachata Vida Dance Company (hereinafter “Bachata Vida Dance Company”).
I/WE AGREE TO RELEASE, INDEMNIFY, AND HOLD HARMLESS BACHATA VIDA DANCE COMPANY, INCLUDING ITS RESPECTIVE DIRECTORS, MEMBERS, EMPLOYEES, TEACHERS, DANCERS, STAFF MEMBERS, OFFICERS, SHAREHOLDERS, SUCCESSORS, SUBSIDIARIES, FACILITIES, FACILITY OWNERS, SUBCONTRACTORS, AND AGENTS, AND THOSE OF ITS AFFILIATES FROM AND AGAINST ANY AND ALL COSTS, EXPENSES, LOSSES, CLAIMS, JUDGMENTS, LIABILITIES, DAMAGES, SUITS, JUDGMENTS, DEMANDS, ACTIONS, OR CAUSE OF ACTION (INCLUDING, WITHOUT LIMITATION, REASONABLE ATTORNEYS’ FEES AND COURT COSTS) (COLLECTIVELY “CLAIMS”) NOW AND IN THE FUTURE, ARISING OUT OF OR RELATED IN ANY WAY TO MY, OR MY CHILD’S OR CHILDREN’S PARTICIPATION IN THE SERVICES. I/WE AGREE TO RELEASE, INDEMNIFY, AND HOLD HARMLESS BACHATA VIDA DANCE COMPANY FOR ANY PROPERTY DAMAGE OF ANY KIND, PERSONAL INJURY, OR DEATH WHICH MAY OCCUR ON THE PREMISES BEFORE, DURING, OR AFTER CLASSES, OR AT ANY COMPETITION, OR WHILE PARTICIPATING IN ONLINE CLASSES. FURTHERMORE, I/WE AGREE TO OBEY THE CLASS AND FACILITY RULES AND TAKE FULL RESPONSIBILITY FOR MY/OUR BEHAVIOR, AND THAT OF OUR CHILD/CHILDREN, IN ADDITION TO ANY DAMAGES I/WE OR MY/OUR CHILD/CHILDREN, CAUSE TO THE FACILITIES UTILIZED BY BACHATA VIDA DANCE COMPANY.

Refund Policy
All sales are final. We do not issue refunds, except under extenuating circumstances of which Bachata Vida reserves the sole discretion and right to determine which cases would qualify for a refund or exchange.
