Liability Waiver Form

 I/we realize that participation in dance classes and activities could involve some possible personal injury.  Despite precautions, accidents and injuries may occur.  By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Bachata Vida Dance Company, LLC dba Bachata Vida Dance Company (hereinafter “Bachata Vida Dance Company”)

 I/we agree to release and hold harmless Bachata Vida Dance Company including its teachers, dancers, staff members, and facilities used from any cause of action, claims, or demands now and in the future.  I/we will not hold Bachata Vida Dance Company liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes or at any competition.  Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Bachata Vida Dance Company.

 Bachata Vida Dance Company PHOTO/VIDEO RELEASE FORM

I hereby grant the {Bachata Vida Dance Company} permission to use my childs likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the {Bachata Vida Dance Company} and will not be returned.

 I hereby irrevocably authorize the {Bachata Vida Dance Company} to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose.  In addition, I waive any right to inspect or approve the finished product wherein my childs likeness appears.  Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

 I hereby hold harmless, release, and forever discharge the {Bachata Vida Dance Company} from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Refund Policy

All sales are final. We do not issue refunds, except under extenuating circumstances of which Bachata Vida reserves the right to deem which cases would qualify for a refund or exchange.
